how to create this application

i am creating a java desktop application where the user will enter his name and age and press next (like given in the image 1.png). And the action would perform like given in the image 2.png

i went through all the tutorials but i didn't found the solution any where. if any one can help me please tell me what code will go back on the "next>>" button (in the pic 1.png) and how to make the window in the pic 2.png
